NYSBA Pro Bono Network-Client Waiver

I certify that the information provided in this application is true and exact to the best of my knowledge.

I have provided all requested information on the intake form application and have uploaded all requested documents to the intake form application, and I understand that it is my responsibility to promptly provide these documents and information to the attorney if requested.

I understand that once the New York State Bar Association (“NYSBA”) refers me to an attorney, the attorney may refuse to accept my case if:

  • My case has no merit.
  • I am not prepared or able to pay the court costs or other expenses necessary for proper handling of my case. Note: Although attorney representation will be pro bono and free of cost, there may be court costs or filing fees associated with your case which you will need to pay. For example, filing fees in Surrogate’s Court may be between $1 and $1,300, depending on the scope and type of proceeding and size of estate.
  • Accepting my case would involve violation of law or the Rules of Professional Conduct.
  • There is any other good reason.

I understand that after an attorney accepts my case, my relationship with my attorney will be governed by the agreement between myself and my attorney, and will be limited to the specific scope of engagement agreed upon between myself and my attorney.

I hereby permit and instruct any attorney, law office staff, legal or social services agency to which this matter is referred by NYSBA to respond by phone or in writing to requests made by NYSBA for information about the facts, progress, or outcome of my case.

I understand that NYSBA does not guarantee it will find an attorney who will accept my case and does not guarantee the outcome or results of that representation.

I release the New York State Bar Association and its officers, directors, members, and employees from any and all claims, losses, and liability arising out of any referral made by or through the COVID-19 Pro Bono Network or by failure to upload or provide requested information or documents for my case.
